I think North America (Us, Canada). However, many of those players just live there, they come from other places (them or their parents migrated in) - Negreanu has romanian roots, Selbst is jewish, etc etc
In our current globalised world, country is not so important anymore, imo.

Here in South America, Brazil is in 1st place, although I don't know if so much for results, but for the number of players that Brazilians contribute to the game itself.
Here in argentina my compatriots have obtained great achievements in the wsop, in PCA, in Europe as well.
We have elite players like Jose Ignacio "nacho" Barberi, Damian Salas, Maria Constanza Lampropulos, Andres "Pappo" Lococco, Ivan "Negrin" Luca, and others.
